To upload a photo of your dog, select the 'Browse' button and locate the photo you wish to submit on your computer.

Please be sure to keep the file size under 1.5 MB.
Click 'Open' after locating the photo.
Then complete the other information, including dog's name, your first name and last initial, your email address, and your phone number.
Once that information is filled in completely, select the 'Submit' button.
